MEDICAL HOME PROGRAMS

Medical Home
With a commitment to children with special health care needs, the Broward County Children’s Services Administration (CSAD) funded an innovative grant supporting enhanced services to children, birth through ten years of age, with diabilities and chronic illnesses through the Center’s Primary Care Clinic since 2002. Medical social work, health education, nutritional counseling, developmental assessments and evaluations are available to those children ineligible for these services through other programs. Medical case managers work closely with the medical staff to assist families in coordinating specialty care and services with other agencies. When necessary, Medical Home staff also helps families with emergency assistance and transportation.

Adolescent Medical Home
In 2007, Broward County’s Children’s Services Administration Division expanded the CDTC Medical Home to establish a program which emphasized the comprehensive care of adolescents from eleven through 21 years of age with developmental disabilities and chronic illnesses. One of the few, if not the only, Medical Homes in the County, the Adolescent Medical Home provides youth-centered medical case management, nutritional counseling and support, health education, developmental and psychosocial services. Teens are also provided with assistance with transportation and emergency assistance as needed.
